Obtaining CNA Certification

Should you wish to modify your mailing address, telephone number and/or e-mail address, please complete the following form. Once we receive the new information, your certification records will be updated. Thank you for keeping us informed.

Proof of identification is required to proceed with a name change. We will accept a current RN licence, driver’s licence, birth certificate, passport, marriage certificate or divorce certificate. Please forward a copy of the required document to the CNA Certification Processing Centre by mail at Suite 210, 1400 Blair Place, Ottawa, ON K1J 9B8 or by fax to 613-237-6684.
