Helping you standardize nursing assessments for electronic health records

The impact of nursing on the health outcomes of patients, clients and residents is demonstrated when nursing has standardized assessments and terminology. While the C-HOBIC project was implemented in two provinces beyond the original work in Ontario, the real goal will be realized only when we see nursing information captured in national databases and nurses in all areas of practice and all parts of Canada can share comparable information. This will support the growth of nursing knowledge and make nursing visible at a health system level.

To further the vision of the C-HOBIC project, and to support the goal of interoperable health records in Canada, the project team is sharing information and resources from the project in this toolkit.

C-HOBIC Fact Sheet for Clinicians

The fact sheet gives an overview to clinicians about the project, how standardized nursing assessments can improve practice and what the benefits will be to Canadians and to our health system.

Mapping Canadian Clinical Outcomes in ICNP

A major deliverable of this project was to have nursing content documented in a standardized clinical terminology and coded in a format suitable for inclusion in electronic health records. With the help of an expert working group, all C-HOBIC terms were mapped to the International Classification for Nursing Practice (ICNP®) Version 1.0. Coding nursing terms to a computer language supports interoperability, consistency and comparability of clinical information that is reflective of nursing practice across health-care system(s) and will facilitate future mapping of these concepts to SNOMED-CT (Systemized Nomenclature of Medicine - Clinical Terms) and to nurses.

Sample Education Presentation

Ontario, Manitoba and Saskatchewan all approached the education of nurses a little differently to respond to the needs of their nurses. There were a number of workshops for nurses and nurse managers on how to use the C-HOBIC patient information to plan for and evaluate care. This educational presentation is an example from one of the workshops that may help you understand ways to begin working with nurses in your jurisdiction.

C-HOBIC Initiative Benefits Evaluation Final Report

Evaluation findings are most positive in care sectors that have been involved in the initiative the longest and have focused resources on assisting clinicians to use the C-HOBIC information to improve their practice. Review this report to learn about the methodology and to read about the potential impact on nursing care in the words of nurses.
